K-3 Basketball Start Dates

• K-1 Boys: March 1, 2008  9:15-10:00

• K-1 Girls: March 1,2008 10:30-11:15

• 2nd-3rd Grade Girls March 1, 2008 1:00-2:00

• 2nd-3rd Grade Boys March 1, 2008 2:30-3:30

 2^nd and 3^rd Grade Basketball Program

 Once again we will be having our 2nd and 3rd grade basketball program. This is open to all boys and girls whose families are registered members of the parish.

 Location: St. Maximilian Kolbe Gymnasium

 Game Days: Friday Nights, Saturday and Sunday Afternoon. You may have two games a weekend.

 Dates:

• March 1, 2008 (evaluations by coaches)

• March 8-9 2008 (team practice day)

• March 14-15-16, 2008 (games begin)

• March 29-30, 2008

• April 4-5-6, 2008

• April 11-12-13,2008

 Practice: Monday –Thursday based on coaches’ availability.

Kindergarten/1^st grade basketball clinic

 Once again we will be having our kindergarten and first grade basketball clinic. This is open to all boys and girls whose families are registered members of the parish.

 Location: St. Maximilian Kolbe Gymnasium

 Day: Saturday Mornings

 Boys Session: 9:15 a.m. -10:00 a.m.

 Girls Session: 10:00 a.m. – 10:45 a.m.

PARENTS NOTES:

(1) Register your child(ren) online

(2) Submit registration fee. (Recommended method is paypal)

(3) Print and complete medical forms. Submit forms to the coach. Forms are online

 COACHES NOTES:

(1) Ensure that all of your CYO mandatory requirements have been met.

(2) Initiate budget. Form is online

(3) Use latest registration list to complete your roster. The roster form is online. Complete all fields. Signatures are required: Coach, Father Schiele and Jim Walsh.

(4) Complete coaches’ card. Signatures are required: Coach, and Father Schiele.

(5) Varsity sports only…Have public school children’s parents sign affidavit. Form is online.

Note: Roster, coaches’ card and affidavit needs to be turned in to the    commissioner.

(6) Direct parents to medical release forms (online) ask that they complete and submit.

            Note: Coach holds onto forms.

(7) Coaches may use online check request to pay for league and referee fees.
